By signing this form, you agree to the terms and conditions set forth in the event

guidelines and if any Slideways Karting Center track rules are broken as

determined by the official, your points and entry fee will be forfeited.

Advertising and Release - In consideration of entering into this event, racers

agree to permit Slideways Karting Center the use of their names and pictures for

advertising and publicity before and after the event, and to relinquish all rights to

any photos or sell the same.

$50 entry fee and registration form must be received by Saturday, August 6th, 2011 at

12:00 noon.  $75 entry fee for any entry received after this deadline.  No personal

checks or credit cards accepted.  Money orders, certified checks, and cash

accepted.  No cash by mail.  Entry form may be mailed without payment as long

as payment is received by deadline.

*Note: We will accept only the first 48 paid entries as drivers. Payment will only confirm your spot.
